What's Actually Missing From a Standard Annual Physical?
A standard annual physical answers one question: is there evidence of disease right now? That is a useful question, and the visit does its job. It is just not the question most people carry into the exam room, which is usually closer to: how am I aging, what is quietly changing, and what should I be doing about the next ten years rather than the next ten days.
If you found your way here, chances are your last physical came back reassuringly normal, and reassurance was not quite what you wanted. What you wanted was a baseline. A real one, deep enough to measure yourself against later and know, rather than guess, which direction you are moving.

What is a standard annual physical actually designed to catch?
The annual physical is a screening visit. Its job is to find established disease and a short list of major risk factors: blood pressure that needs treating, cholesterol high enough to act on, blood sugar past the diabetes threshold, cancers with recommended screening ages. Each item is anchored to population-level cutoffs, so the visit is built to flag the moment a problem becomes a diagnosis.
None of this is a criticism of the physician running it. Primary care appointments are brief because of how the system is structured, not by the physician's choice, and the screening checklist genuinely earns its keep. The limitation is scope. The visit asks whether you have crossed a line, not how fast you are moving toward one, and prevention, in the fullest sense, lives in that second question.
Why can labs be normal when you don't feel optimal?
Reference ranges are wide by construction. They describe where most of the tested population lands, not what thriving looks like. A person can sit at the unfavorable edge of half a dozen ranges at once, each result technically normal, while the overall pattern points somewhere a screening review was never designed to look. People in that position often describe the same thing: the numbers say fine, the mornings say otherwise.
A snapshot also hides trajectory. A glucose value that is in range but has drifted upward across years of physicals tells a different story than the same value holding steady, yet a single visit treats them identically. And some of the places where change shows up earliest, fasting insulin, inflammatory markers, the particle-level detail behind a cholesterol number, are simply not on the standard panel. That is the gap between screening for disease and measuring health, and it is why "normal" can be true and still incomplete.
What does a biological age test actually measure?
Underneath all of this sits a simple question: what is my biological age, and is it running ahead of my birthdays? Chronological age counts years. Biological age tries to estimate how the body is actually wearing. An epigenetic age test reads chemical marks on DNA that shift in broadly predictable patterns across a lifetime, and an epigenetic clock converts those patterns into an age estimate. A phenotypic age calculation combines ordinary blood markers instead, estimating how old your physiology is behaving. A biological age blood test drawn and interpreted by a physician sits between the two: familiar chemistry, read for what it says about aging rather than only disease.
Is a biological age test worth it? As a single number arriving alone in an inbox, often not. Different clocks weight different signals and can return different answers for the same person, so the first report is less a verdict than a starting point. The value compounds with repetition: a baseline now, the same biological age assessment repeated later, and the direction of travel in between. Read inside a full clinical picture, that trajectory answers the question people are really asking, which is whether they are aging faster than they should be.
How do you actually measure healthspan, not just lifespan?
Healthspan is the stretch of life lived with full function: strength, energy, clear thinking, independence. Lifespan gets counted for you. Healthspan has to be measured on purpose, mostly with functional tests rather than blood draws. Cardiorespiratory fitness, captured as VO2 max, shows how well heart, lungs, blood, and muscle perform together under real demand. Body composition testing separates muscle from fat and shows where the fat sits, which matters far more metabolically than the number on a scale. Strength and balance measures round out the picture, tracking the capacities that determine what your later decades actually feel like.
VO2 max and body composition testing are conspicuously absent from a standard physical, and they are precisely what people focused on longevity most want to know. They are also unusually satisfying to measure because, unlike a family history, they respond to training. They move when you work on them, and a baseline is what lets you see it.
What does a real baseline health workup include?
A genuine baseline reads several systems at once. Cardiovascular risk beyond the standard cholesterol panel, including particle-level lipid detail and lipoprotein(a), a largely inherited marker worth measuring at least once. Metabolic health beyond a fasting glucose, starting with fasting insulin, which often shifts years before glucose does. Inflammation, through markers a routine panel usually skips. The hormonal picture in full, thyroid and cortisol and sex hormones, read as context for everything else rather than in isolation. Then the functional layer from the previous section, body composition and fitness, sitting alongside the bloodwork.
The point of a deeper bloodwork panel is not more data for its own sake. These systems change together, in patterns, and a physician reading them as one picture can notice a pattern forming years before any single value would trip a screening threshold. It is also the practical difference between an executive physical and a longevity physical: one gathers more tests into a single day, the other is organized around what your results, taken together and tracked over time, say about the decade ahead.
What should you ask for at your next visit?
Keep your physician. A deeper baseline is not a replacement for primary care, and the goal is a better conversation, not a different allegiance. Useful questions to bring: which systems did my panel actually assess? Can we look at my results as a trend across years rather than one visit at a time? Has my lipoprotein(a) ever been measured? What would we measure differently if the goal were optimization rather than screening? The trend question alone often changes what the visit surfaces.
Some of those questions outgrow what a short appointment can hold, and that is where a physician-led longevity workup fits.
